National Diabetes Prevention Programme

We would like to advise our patients that Amwell Group Practice has engaged with the National Diabetes Prevention Programme (NDPP) centralised text message project.

This project will support high-risk patients into the NDPP through a centralised text message/letter approach. The project has been through NCL CCG governance processes and the programme is now ready to start. High-risk patients who are eligible for the NDPP and have a diagnosed long-term condition will be contacted by SMS (consent permitting) or by letter if not, and invited to participate in the Programme should they wish to.
