Care co-ordinator
What is Care Coordination?
Care Coordinators help you navigate the health and care system, ensuring you connect with the right teams at the right time. They assist you in managing your healthcare needs by answering questions, making and managing appointments, and providing clear information to help you make informed choices about your care.
If you’re housebound, Care Coordinators can also visit you at home to support your health needs.

Who Can Use the Care Coordination Service?
You may be eligible for the Care Coordination service if you:
Frequently attend the hospital (inpatient or Emergency Department)
- Are registered with one of our GP surgeries
2. Are 18 years or older and registered with an Islington GP
3. Frequently attend the hospital (inpatient or Emergency Department)
4. Have two or more of the following concerns:
- Social Issues
- Physical Health Issues
- Mental Health Issues

Social prescribing link worker
What is a Social Prescribing
Our social prescribing service strives to empower you to take charge of your health and well-being.
The GP Practice Healthcare Team provides Social prescribing services through a Link Worker. Link Workers assists you in discovering personalised methods to boost your well-being. The Link worker will help you find ways to improve how you feel in a way that suits you.
The Link Worker helps individuals find local support and activities for their wellbeing. They work with Islington Council, NHS, and the voluntary sector to connect you to tailored resources. They offer emotional support, practical help, and advice.
You must be over 16 years old to use the service.

How do I get an appointment with a Link Worker?
Complete an eConsult form or contact your GP practice
A healthcare professional or reception staff member from your GP Practice team will provide a referral for you.
The Link Worker will reach out to you via phone within 5 working days from the referral date to discuss the referral. During the call, the Link Worker may suggest meeting you either at your GP practice or another local venue.
To provide you with the best possible care, the Link Worker will need access your complete medical records, but only if you grant consent during the initial referral process.

First contact physiotherapist
What is a First Contact Physiotherapist?
A First Contact Physiotherapist (FCP) provides immediate care for musculoskeletal (MSK) problems, such as joint pain, back pain, and sports injuries. Patients can see the FCP directly without needing a GP referral.

Key Responsibility
Assessment and Diagnosis: The FCP assesses, diagnoses, treats, and manages MSK problems.
Medication: They can prescribe medication within their scope of practice.

Referrals: The FCP can signpost or refer you to other health and care services as needed.
Investigations: The FCP can request and progress any necessary investigations to facilitate your care.

What is a Paramedic?
Our paramedic provides urgent care and support within the GP practice, focusing on patient assessments and managing minor health issues.
